Interim Lead Complex Case Manager (Learning Disability)– NHS ICB, West Midlands – ASAP Start – 6 Months – Inside IR35 – £350-£400 Day Rate – Hybrid working

We are currently working with our Client, an NHS ICB based in the West Midlands, who are looking for an Interim Lead Complex Case Manager (Localities).

The role is looking to start ASAP and will be for 6 months. The Interim Lead Complex Case Manager job will ideally be performed on a full-time basis. In terms of remuneration, this role falls inside IR35 on a £360 Day Rate. In addition, the successful candidate would be expected to be available for patient visits three days per week, while the rest of the role can be worked remotely.

Job Role - Reporting to the All Age Continuing Health Care Lead, key duties will include;

  • Working jointly with health, social care and independent sector colleagues to ensure care management processes are appropriate and being able to present assessments when required
  • To follow the process laid out in the National Framework for NHS Continuing Healthcare; to include organisation, coordination and participation in MDT eligibility recommendations to be verified by key decision-makers
  • Undertaking assessments for adults over 18 years of age with complex health needs as well as reviewing and monitoring of care where NHS Continuing Healthcare funding is in place, including retrospective and appeal cases

The Person - Candidates will need to be educated to a degree level or equivalent level of experience working in a specialist area. Candidates will also need to have a registered nursing qualification to apply.
