&lt;p&gt;As part of our large scale change programme, we have developed the &amp;lsquo;Sunderland Way of Working&amp;rsquo;,&amp;nbsp; focused on making a step change on the way public services are delivered. To ensure our Health, Housing and Adult Services directorate is best placed to deliver on these changes, we are looking for a Head of Care and Support to join this team.&lt;/p&gt;&lt;p&gt;This postholder will be responsible for developing a modern care management structure for adult social care and housing, encompassing all provision across the city. You will need to work closely with commissioners and providers, developing and quality assuring the market in Sunderland to meet the needs of changing agendas, such as personalisation. You will also need to put in place a single community based support system, focused on health and wellbeing of local people.&lt;/p&gt;&lt;p&gt;The exact structure beneath this post is currently under review, but it will cover the services that the directorate either&amp;nbsp;provides directly or commissions. These include home support, extra care sheltered housing, independent living homes for people with learning disabilities and mental health needs, health impact assessments and community equipment store.&lt;/p&gt;&lt;p&gt;Whatever your background, you will need to set the long-term vision for how services will be shaped to meet future demands, allowing all our citizens to live as independently as possible. Partnership working will also be key, working with colleagues both internally and externally to successfully change the way that services are delivered to our communities.&lt;/p&gt;&lt;p&gt;The exact structure beneath this post is currently under review, but there will be three core divisions: Reablement; Initial Advice and Assessment; and Complex Needs/ Specialist Service Areas.
