The Health and Social Care Directorate provides the full range of community and healthcare services to our 90,000 residents and we are looking to identify a highly experienced, driven and resilient individual to take on the newly created role of Managing Director of Community Services.

With responsibility for leading the development and delivery of an integrated health and social care function for adults and children, you will work closely with the Chief Officer for Health and Social Care and her senior team to ensure that service provision meets the needs of all residents and visitors. With a focus on safeguarding, prevention and helping people to live independent meaningful lives, an early priority will be to lead a strategic review to respond to recent external reviews and focus on service improvement. You will also ensure that governance and effective management of risk are at the heart of everything we do.

With a background and professional qualification in social work your impressive track record will evidence success in service improvement and transformation. With experience of leading social care functions of considerable complexity and working within, or alongside, the healthcare sector, you will be able to provide direction and supportive leadership to a large multi-disciplinary team.

Head of Care and Support

As part of our large scale change programme, we have developed the ‘Sunderland Way of Working’, focused on making a step change on the way public services are delivered. To ensure our Health, Housing and Adult Services directorate is best placed to deliver on these changes, we are looking for a Head of Care and Support to join this team.

This postholder will be responsible for developing a modern care management structure for adult social care and housing, encompassing all provision across the city. You will need to work closely with commissioners and providers, developing and quality assuring the market in Sunderland to meet the needs of changing agendas, such as personalisation. You will also need to put in place a single community based support system, focused on health and wellbeing of local people.

The exact structure beneath this post is currently under review, but it will cover the services that the directorate either provides directly or commissions. These include home support, extra care sheltered housing, independent living homes for people with learning disabilities and mental health needs, health impact assessments and community equipment store.

Head of Personalisation

Personalisation is a huge change agenda in adult social care and this postholder will be responsible for developing, implementing and managing this for Sunderland City Council. You will need to support and lead our social care workforce during what will be a challenging time of transformation, ensuring that they are equipped to deliver on this agenda and that they understand what this means for service delivery. Partnership working will also be key, working with colleagues both internally and externally to successfully change the way that services are delivered to our communities.

The exact structure beneath this post is currently under review, but there will be three core divisions: Reablement; Initial Advice and Assessment; and Complex Needs/ Specialist Service Areas. A Care Brokerage Service is also being established.
